The HCR only supports the ability to return one reason for the decision for the Health Care Service Review action. Often there are multiple reasons for the decision that has been made. There is a need to be able to report back multiple decision reason codes. For example, in Medicaid, it is required that multiple reasons for denial be reported back in order for the physician or Medicaid recipient to use if they choose to appeal the decision.
